Workflow criteria to formula?

Hello!  I have a workflow rule that should trigger an outbound message. The criteria are pretty extensive and I'm finding that the outbound message isnt triggering when it should. See screenshot below. For example, Ive confirmed that item 7 in the case contacts object is definitely true, but it triggers the workflow anyway even though other criteria are met.

I've seen this happen before in lead assignment rules and had to convert the criteria to a formula instead. Do you think that needs to be done here? 

Otherwise, is there a way that I can generate a log file of the workflow that I can use to troubleshoot?

Hi Gary you filter log says to fire the workflow if 7, 8, 9, 10, OR 11 are true.  This means if #7 =True, but #8= false, then the workflow fires. 

I think maybe working on your logic some should produce the outcome you are looking for, based on reading your criteria, I think the following might work:

(1 AND 2 AND 7 AND 8 AND 9 AND 10 AND 11 AND (3 OR 4) AND (5 OR 6))
