Nesting the OR function with the ISCHANGED function

I am trying to develop a workflow rule so I will receive an email anytime someone changes a key contact from an account. How do you use the OR function with the ISCHANGED function, so I don't need to make a rule for every field that may change? Below is the last formula I tried, which may totally be off. I am getting the following Error: Function ISCHANGED may not be used in this type of formula.

OR(
ISCHANGED(Operations_Leader__c),
ISCHANGED(Financial_Leader__c),
ISCHANGED(Operations_Leader__c),
ISCHANGED(Clinical_Leader__c),
ISCHANGED(IT_Leader__c),
ISCHANGED(EHR_HIT_Leader__c),
ISCHANGED(Quality_Leader__c),
ISCHANGED(PCMH_Leader__c),
ISCHANGED(PI_Leader__c),
ISCHANGED(Research_Leader__c),
ISCHANGED(Data_Leader__c),
ISCHANGED(HCCN_Contact__c))

You are on the right track. You will have to include the name of every field you are looking for.

Now for the error, choose the second option in the Evaluation Criteria - created and every time it's edited [2ND OPTION]
