Process builder flow failing

I have a process builder which create campaign member entry on a lead when the lead comes from Marketo/Drift. 
the process builder create a record based on the condition, set field values as follows. This is works fine for me, but I am keep getting email alerts with subject- Error Occurred During Flow "Drift_campaign_mapping": This error occurred when the flow tried to create rec.....

I believe thse triggers when there an rule met and the same creates a duplictate entry.
