Workflow trigger when a field value changes

Can someone help me create a workflow to send an email alert when a field is modified?

Pick list Field Name = "report delivery preference"

pick list values:
  • Fax Only
  • FedEx Overnight Only
  • Fax and FedEx Ground
  • Fax and FedEx Overnight

I would also like to create another workflow to send an email when a fax number changes. 

Field Name: Fax

When is your WFR set to evaluate?  ISCHANGED can only be used if it's set to "Every time the record is created or editied"

How would I set up a WFR to trigger off an address change. Address, City, St or Zip are all separate fields. 

I have strong faith that you will no doubt have an answer. :)

Evaluate Rule: Every time a record is created or edited

WFR Trigger Formula:

OR(
 ISCHANGED( Street ),
 ISCHANGED( City ),
 ISCHANGED( State ),
 ISCHANGED(  PostalCode  ),

Stevemo, i have run into a problem where the WFA is triggered when a value changes from null to a new value. For example, when a record does not have a fax entered and is entered at a later time the WFA is triggered.

Is there was a way to set it up so the WFA is only triggered when a fax number changes from one value to another, e.g. 415.555.5555 to 415444.4444 and not null to 415.4444.4444?
